CLINICAL STUDY: 
FOR NCM 104 Maladaptive Patterns of Behavior Students: 
COGNITIVE DISORDERS

     Client N.M age 70 has been diagnosed with Alzheimer's disease for almost 5 years.  During the first few years of her disorder, she was able to function effectively at home with the assistance of her husband.  When her husband died, she went to live with her daughter.  While she was staying in her daughter's home, her memory and cognitive functions deteriorated rapidly.  She became more and more withdrawn during the day and began staying awake at night.  During this day and night reversal patterns, she is found wandering in the garden several times.  She became less and less socially interactive and started refusing food and water, even her favorite peanut butter jelly sandwich.  With this, her daughter can no longer manage her and went to her family doctor to discuss her mother's condition.  Client N.M. was examined by the physician and found to have lost 25 lbs and was severely dehydrated.  Due to her condition, she was admitted in the hospital for nutritional support and fluid replacement.  

     While she is in the hospital, her cognitive problem continued to deteriorate and it was already necessary for her to be fed through tube feeding.  She started with nasal feeding and was eventually placed on gastric feeding.  Due to the deterioration of her cognitive functioning in the hospital, the family decided to bring her home.  As a home care nurse, a referral from the hospital was given to you to inform the family members of home health teachings.
